PARTICIPATION WAIVER AND RELEASE OF LIABILITY AGREEMENT In consideration of being allowed to participate in any way in the sport and activities of [Your Name/Business Name], I, the undersigned, acknowledge, appreciate, and agree that: Release of Liability: I hereby release and waive my rights to sue Drip Hoops Miami LLC, its officers, employees, coaches, volunteers, agents, representatives, and any other individuals associated with Drip Hoops Miami LLC (collectively "Released Parties") from any and all liability, claims, or demands arising from injury, loss, or damage to my person or property, whether caused by the negligence of the Released Parties or otherwise. Indemnification: I agree to indemnify, hold harmless, and defend the Released Parties from any loss, liability, damage, or cost they may incur due to my participation in the Drip Hoops Miami LLC activities, whether online, in-person, or otherwise. Assumption of Risk: I willingly and freely assume all risks associated with my participation in Drip Hoops Miami LLC activities, including but not limited to, risk of injury, illness, and damage to my person or property. I understand the specific risks associated with basketball. Emergency Medical Treatment: I consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during the activities. Media Release: I grant Drip Hoops Miami LLC the right to take photographs or video footage of me during my participation in activities. I authorize Drip Hoops Miami LLC to use such media for any lawful purpose, including but not limited to, publicity, illustration, advertising, and web content. Compliance with Policies: I agree to comply with all rules, regulations, and safety procedures established by Drip Hoops Miami LLC for my well-being and to minimize the risk of injury or contraction of diseases. Severability: If any clause or provision set forth in this waiver is determined to be illegal, invalid, or unenforceable under present or future law, then it shall be severable without affecting the enforceability of all remaining clauses or provisions. Acknowledgement of Understanding: I have read this waiver of liability, fully understand its terms, and understand that I am relinquishing substantial rights by signing it. I acknowledge that I am signing this agreement freely and voluntarily, and intend my signature to serve as confirmation of my complete and unconditional acceptance of the terms, conditions, and provisions set forth. If the participant is a minor, I certify that I am the participant's parent or legal guardian and I make these representations and provide my agreement to this waiver and release on the participant's behalf. *
